POSITION SUMMARY Coordinates the development, completion and transmission of the resident assessment in accordance with the regulatory requirements; reviews and manages reimbursement in compliance with SunBridge policies and procedures. Responsible to case manage residents covered by either Medicare and Managed Care insurance programs. Essential Position Duties * Conducts and coordinates the completion of the resident assessment (MDS), Triggers and RAPs in accordance with current rules, regulations and guidelines that govern the resident assessment. * Coordinates and supervises the development of full, quarterly and comprehensive assessments and care plans of each resident within required time frames. * Develops the schedule of activities required for the resident assessment and plan of care including participation by the interdisciplinary team, external partners and family members. * Assists the Director of Nursing Services and relevant supervisors of other departments in ensuring that personnel who provides care are aware of the residents’ care plans and that nursing personnel refer to the residents’ care plans prior to administering daily care to the residents. * Completes daily records review of medical and managed care residents to identify change of condition. * Conducts and coordinates facility daily and weekly PPS meetings and ensure timely submission of clinical data to business office reimbursement processes. * Participates in the monthly triple check meetings. * Coordinates discharge planning for skilled nursing and rehab residents in conjunction with social services. * Reviews nursing documentation to ensure state, federal and third party payer guidelines are met to support skilled coverage. * Collects pertinent information for benefits, verification/authorization and reauthorization for coverage. * Maintains physician certification and recertification files. Ensures certs/recerts are done timely and filed in the medical record. * Performs administrative duties such as completing medical forms, reports, evaluations, studies etc. as necessary. * Participates in the planning, conducting and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment functions of the facility. * Participates in the facility Quality Assurance and CQI process. Serves on committees and develops/implements improvement. * Reviews Quality Indicator report and Resident Specific Profile no less than monthly. Analyzes and addresses flagged areas in conjunction with the interdisciplinary team. Provides Administrator and Director of Nursing Services with the QI Report and an analysis of flagged areas. * Attends/participates in educational programs to maintain knowledge current with RAI/PPS regulations. * Performs all other tasks or duties as assigned. Professional Experience/Educational Requirements * Registered Nursing degree; BSN preferred. * Demonstrated experience in long-term care. * Demonstrated proficiency in RAI/PPS processes. Certification/Licensure * Current RN license in state of employment.
