    When dealing with an addict who refuses treatment, you have to realize that you are powerless over their decisions.  You have to learn how to detach yourself from their problems.  The addiction that they have is creating a delusional euphoria.  There is no worse feeling than watching someone you know slowly kill themselves and refuse to seek  help. 
    If they absolutely refuse to go to an inpatient treatment center, then maybe it'd be better to take them to a physican, possibly their own primary care provider.  Treatment advice coming from a professional who cares about them could have a bigger impact.  Next, set up a meeting with their family and come up with an intervention strategy, with the general idea to convince them if they don't get help then they're probably going to die.  Let them know that you only want to support them in their recovery.  Convince them in a loving way so that they feel like they are finally getting the help they need.

Whether or not an addict refuses treatment, depends a lot on what they're addicted to.  Someone addicted to crystal meth or opiates is going to be a lot harder to convince because they're chemically dependant, but someone addicted to porn and gambling needs a different type of convincing.  All addictions are dangerous and strenuous on family members.  It's best to have interventions as early as possible though.

I think every addict just has to have that eye opening moment in their lives where they realize that they've hit rock bottom and they need to stop or they could lose their lives. It could be a ruined marriage or relationship, maybe losing someone they love,or just a bunch of things put together that just makes them realize. But I believe without that moment its going to be very hard for them to reach out for help or to continue with their recovery.
